How to Access Point and Click
CSUB Services for Students with Disabilities (SSD) is pleased to announce that we are transitioning to a paperless office. We are now using Point and Click for intakes, annual updates, accommodation letters and much more. You will now complete all necessary forms online prior to any scheduled appointment. Beginning Fall 2021, you will access your Accommodation Letter via your secure student portal. We are offering both in-person and Zoom meetings at both CSUB's main campus and AV location. For the health and safety of all students and in adhering to Kern County and Los Angeles County Public Health guidelines, we are limiting the number of people in the office at any given time. Please schedule an appointment or call ahead of time, if possible. Note: We will continue to require masks and social distancing of 6 feet.
Steps for Scheduling an Appointment with CSUB SSD
New Students
- Go to: https://health.csub.edu/home.aspx
- You will be prompted to input your CSUB Net ID and Password
- After this, you will go to a page that asks you information to confirm your identity by providing DOB.
- At this point, you will be on the Student Health Services and Services for Students with Disabilities Student Portal
- Click 'Messages' on the left-hand side of the page. Click 'New Message' and choose SSD Initial Intake Form. Complete and Click 'send' at the bottom of the page.
- Go back to messages and click 'New Message' and choose SSD Consent Forms. Complete and click 'send' at the bottom of the page.
- For students requesting services due to a Physical or Psychological Disability, you will need to provide SSD with a completed Verification of Disability Form. To access the form, click here. Once it has been completed, scan and email to:ssd@csub.edu (Main Campus); or mbrooks4@csub.edu (CSUB Antelope Valley Campus).
- Once all forms have been completed, contact SSD to schedule your appointment: 661- 654-3360 or ssd@csub.edu (Main Campus); 661-952-5061 or mbrooks4@csub.edu (CSUB Antelope Valley Campus).
Returning Students (For annual updates)
- Go to: https://health.csub.edu/home.aspx
- You will be prompted to input your CSUB Net ID and Password
- After this, you will go to a page that asks you information to confirm your identity by providing DOB.
- At this point, you will be on the Student Health Services and Services for Students with Disabilities Student Portal. Click on 'Messages' on the left-hand side of the page. Click on 'New Message' and choose SSD Annual Review. Complete the enclosed consent form and click 'send' at the bottom of the page.
- Your SSD Counselor will inform you if there are additional forms that need to be completed.

Day of Appointment
- Covid-19 Screening Evaluation (Day of Your Appointment): Students must complete a pre-screening processbefore coming to campus. If you do not complete this pre-screening process, you will not be cleared to come
to campus and you will need to reschedule your appointment. If you bring an attendant, family
member or friend with you, they must follow the prescreening process too.
- To complete the pre-screening questionnaire, open the CSUB Mobile app; click on the COVID-19 Self Screening icon. The results will stay on the app until the end of the day. Be prepared to show this confirmation to campus personnel upon request.
- On Campus Protocol (Time of Your In-Person Appointment): For your appointment, please
be aware of the following:
- You are required to wear a mask during your appointment. SSD does not provide masks; if you do not have one, your appointment will need to be rescheduled.
- Your appointment time is NOT flexible; if you are more than 15 minutes late, your appointment will be rescheduled.

- Be seen for your appointment.

All Students
After you have met with your SSD Counselor, you can find your updated accommodation letter on the Student Health Services and Services for Students with Disabilities Student Portal (different from the MyCSUB portal)
To access the portal:
- Go to: https://health.csub.edu/home.aspx
- You will be prompted to input your CSUB Net ID and Password
- After this, you will go to a page that asks you information to confirm your identity by providing DOB.
- At this point, you will be on your student portal.
- To access your accommodation letter, click on "letters" found on the left-hand side of the page. You will be able to download and save a copy of your accommodation letter to your computer. Please save the letter as a .pdf file. For example, AL23-24.pdf. Don't forget to provide a copy of the current accommodation letter to your instructors at the beginning of the semester.
