Academic Petition - Multi-Purpose |
Graduate Students will initiate this petition by completing the fillable form. Leave of Absence Request - If you plan to be absent from the university for two or more terms, you must request a leave of absence to preserve your catalog rights. Repeat of Course with Forgiveness - Retaking a course and replacing the previous grade with the new grade. Consult catalog for guidelines. Extension of Time Limit for Completing Degree/Program Requirements - Graduate students should check with the appropriate graduate program director to discuss the specific time limit requirements of the respective graduate degree program since some programs may have more stringent requirements. Academic Notice |
The Graduate/Credential Program Director or faculty advisor will initiate this form as needed - You can submit the request by completing the fillable form. Academic Notice Form - Students admitted to a graduate program may be subject to academic notice when their performance is judged to be unsatisfactory for the scholastic and/or professional standards established by the individual graduate program. A post-baccalaureate classified student shall be subject to academic notice at the end of any academic term where the student's grade point average for all course work attempted at CSUB after admission as a post-baccalaureate student falls below 3.0 GPA. |

Academic Disqualification |
The Graduate/Credential Program Director will initiate this form as needed - You can submit the request by completing the fillable form. Academic Disqualification Form - A graduate student shall be subject to academic disqualification if, while on academic notice, the student fails to satisfy the conditions of probation and attain a 3.00 GPA or when their performance does not improve to the scholastic and/or professional standards established by the individual graduate program. A post-baccalaureate classified student shall be subject to academic disqualification if, while on academic probation, the student fails to earn sufficient grade points necessary to be removed from academic probation status and attain a 3.0 GPA. |

Graduate Course Substitution or Waiver |
The Graduate/Credential Program Director/Advisor will initiate this form as needed - You can submit the request through AdobeSign or by completing the fillable form. Graduate Course Substitution or Waiver - The graduate program director/advisor will use this form to initiate a request for a graduate course substitution or waiver or GWAR waiver. All graduate course substitutions or waivers must be approved by Academic Programs and then by the Registrar. Information from the program director/advisor and student is required; once submitted, the program director/advisor listed above will receive the electronic form at the email address they entered. |

Graduate Student Grievance Form |
Graduate Students will initiate this form and send it to the respective Graduate Program Director - You can submit the request through the fillable form. Graduate Student Grievance Form - After exhausting all informal complaint procedures, graduate students can file a written grievance by completing the Graduate Student Grievance Form and sending the completed form with supporting documentation to their graduate program director for their signature. The program director/respondent shall forward the form to their dean/associate dean acknowledging that informal efforts have been exhausted to resolve the issues being grieved. The dean/associate dean shall forward the grievance form to Martha Manriquez (mmanriquez5@csub.edu) for processing. The form will then be submitted for signature to the Associate Dean of Graduate Studies. |

Personal Data Change Form |
Graduate Students will initiate this form and send it to the Office of the Registrar - You can submit the request through AdobeSign or by completing the fillable form. Personal Data Change Form - Graduate students will complete the form, specifying what information needs to be changed. Acceptable documentation verifying the changed information must be provided; the form itself lists the types of acceptable documentation. |

Request for Incomplete Grade |
Graduate Students will initiate this form and send it to the respective Graduate Faculty - You can submit the request through AdobeSign or by completing the fillable form. Request for Incomplete Grade - An incomplete may be assigned when a student has been doing satisfactory work in a course but, for unforeseen reasons judged appropriate by the instructor, should be permitted additional time to complete the requirements. An incomplete grade is not to be given because the student is failing the course. Each incomplete grade must be removed by the end of the subsequent term unless the instructor has set a later date for stated reason. This date may not exceed one year from the end of the term in which the Incomplete was incurred. |

Incomplete Grade Extension Request |
Graduate Faculty will initiate this form and send it to the Office of the Registrar - You can submit the request through AdobeSign or by completing the fillable form. Incomplete Grade Extension Request - This form is to be submitted by the instructor only and a separate form must be filled out for each student. Each incomplete grade must be removed by the end of the subsequent academic semester unless the instructor has set a later date for stated reasons. This date is to be no more than one academic year from the end of the semester in which the incomplete was incurred. |
