Employer-Paid Life and AD&D Insurance
The CSU provides Basic Life and AD&D (Accidental Death and Dismemberment insurance) insurance at no cost to you if you are an eligible employee. The Basic Life and AD&D insurance becomes effective on the first day of the calendar month following the date you become eligible. For answers to commonly asked questions, exclusions, limitations and reductions, beneficiary designations and claims, please refer to The Standard.

Employer-Paid Long-Term Disability (LTD)
The CSU provides employer-paid Long-Term Disability insurance (LTD) to eligible employees within specified employee bargaining units. The Standard Life Insurance Company (The Standard) is the insurance carrier for this benefit.
The following units are subject to a 6-month waiting period:
- Physicians (Unit 1)
- Faculty (Unit 3)
- Academic Support (Unit 4)
- Management Personnel Plan (MPP)

Recognizing that individuals have different financial needs, The CSU provides eligible
Employees the opportunity to apply for voluntary life, accidental death and dismemberment
(AD&D) insurance and long-term disability for themselves and their families at competitive
group rates. This is in addition to any employer-paid Basic Life insurance already
in force. Employees may apply for a Voluntary Life Insurance plan at any time, however, if an
employee applies within 60 days of hire no medical certification is required for up
to the guaranteed issue amounts of $150,000 for employees, $50,000 for Spouses/Domestic
Partners and $20,000 for Dependent Children. Visit the website to calculate the amount of life insurance coverage you may need

by going to the Premium Calculator. Visit The Standard or contact The Standard customer service at 1-800-378-5745. The Accidental Death & Dismemberment plan, offered by The Standard, pays a benefit to the employee or his/her selected beneficiary in the event of death
due to an accident or dismemberment at a benefit level selected by the employee. Offered
to all regular benefits-eligible CSU employees. Coverage for eligible dependents is
available at an additional cost. Eligible employees can enroll at any time during
the year and will not be subject to Evidence of Insurability. Visit The Standard or contact The Standard customer service at 1-800-378-5745. The voluntary Long Term Disability plan, offered by The Standard, provides loss of
income protection (up to 60%) due to a qualified disability. This benefit varies by
employee group. For eligibility details, review the Benefits Summary. Two plans are offered, one with a 30-day and one with a 90-day waiting period. is an affordable way to make up the difference between what your medical insurance
covers and what you’d owe out of pocket if you or a family member were to be diagnosed

that come with an injury—whether you’re coping with a broken arm or recovering from
a serious car accident. Accident Insurance Plan Benefit Guide (PDF) Visit The Standard or contact The Standard customer service at 1-800-378-5745. If an employee should die before retirement, CalPERS may pay the designated beneficiary
a $5,000 tax-free death benefit. Depending on the employee’s length of service, the
beneficiary may receive additional benefits. CalPERS may also pay the employee’s beneficiary
