In doing business with ITS, you will occasionally need to submit a form. For your convenience, most forms regarding the services we provide are located here. The forms are organized by category and listed in alphabetical order. Choose the appropriate category for the associated forms.

Department Shared Folders
Department Share Access
The File Share Access Manager allows each department to make share access changes without contacting the Help Desk. Each department has a designated person to manage access to their share folder. If a department needs to add or change the department designee, submit a Share Folder Access Designee form. If you need access to a department share, please contact the department in question.
New Main Folder Request
To request a new main folder or change the name of an existing folder on the V drive, please call the Help Desk at 661-654-2307. When you contact the Help Desk, please let them know the folder name, the designee that will manage folder access, and the new folder's owner (ie. MPP/Dean/VP).
Note: Subfolders should be created by the department using Windows Explorer.
New Group Request
File Share Access Manager Designees can request new access groups for main and subfolders. For example, if you have a new or existing subfolder that needs to be limited to a few people instead of the entire department (the default), you may request a new group be created. To request a new group, contact the Help Desk at 661-654-2307. When you contact the Help Desk, please specify the folder name, it's location (or path) on the V drive, and the type of access the group(s) needs, such as Read or Read/Write.
Enterprise Document Management (EDM) - OnBase Access Forms
Access to the EDM system will allow the requester to view or manage documents for various campus departments. This access may include viewing, importing/scanning, searching/retrieving, and workflow processing for department documents. Other software features may be available to manage department documents, depending on configured permissions.
This form must be completed to gain access to EDM System (OnBase). Please complete the form and follow the business process defined in the form for granting access.
CSUB Solutions Consulting Forms
Solution Request Form
This form must be completed in order to request a new product or service. Additionally, this form may be required if there is a request for a significant change to an existing product or service. Examples of when this form is required are:
- A new request to purchase a system
- A new request to purchase software
- A need has been identified and a solution needs to be defined
- Going from on premises to cloud based hosting
Once you submit the form below a response for next steps will be provided back to the requester within two weeks. Please note, response to solution request is independent of the solution implementation.

Project Management Forms
Project Charter Request Form
The project charter request form must be completed in order to request significant implantation changes related to ITS services, which includes new product or service or enhancing or modifying existing service. Examples of when this Form is required are:
- A request to implement newly procured solution
- A request to implement enhancements to existing system
- A request to identify & implement a solution for a defined business need
Once you complete this document, please send it as an attachment in an email to the ITS project management office, as well as to the identified project leadership team. A response for charter review will be provided back to the requester & leadership team within two weeks of submission.

Project Closure Summary Form
The Project Closure Summary Form facilities the transfer of knowledge from project team to operations/support team. ITS PMO office recommends using the document template, as part of the project closure.
This document provides the summary of deliverables related to the goals set forth by the project charter during project initiation. In addition, the document may cover unresolved items, upcoming known fixes/ features and future considerations for the customer to consider.
This document is expected to be completed by the project team and/or project manager, and sent to sponsor(s) of the project for approval and sign-off. Upon sponsor's approval of summarized project closure document, the project status will be updated to "Closed" in the ITS PMO dashboard.
As a best practice, please consider sharing the updated document (or document link) via email to the required recipients for reviewing, providing the feedback, and revising the document as needed. In addition, consider including ITS program management office in communications for providing guidance.
Work at Home Software
The CSU licensing agreement with Microsoft permits current CSUB staff and faculty to install one copy of covered programs on a home computer. This agreement is limited to faculty and staff only, who are the primary user of the home computer. Additionally, the software must be used for work-related purposes.
Work-At-Home Software Basics
Under this program, you will receive a CD with the selected software. To defray costs associated with producing the CDS, you will pay a nominal fee for software. To participate in the Work-At-Home Software Program,
- You will need to complete the Work-At-Home Software form
- Provide payment for the associated fees.
